Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
At what point can/do you make a diagnosis of MTC? Calcitonin is not extremely high, but tripled in 6 months. FNA of two nodules by very inexperienced doctors was negative. US revealed 4 nodules with concerning features. Calcium stimulation test sent my number over 100. All other thyroid levels are normal. History includes Pheochromocytoma, CCRCC, and Prostate cancer. Genetic testing of 24 variations were all negative.
Now, most of the multinodular thyroid carcinoma represent unilaterally but it can be bilateral also. FNAC by not experience doctors may not be perfect, plus you are also having a history of pheochromocytoma and prostatic cancer. So it's chances with multinodular thyroid cancer can be higher. So have a checkup with other oncologists which will better diagnose your modules. Blood tests with calcitonin and level of calcium may not be consistent until FNAC reveal MTC.
Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Nirav Nimavat,
General & Family Physician
